I hereby authorize Frontier Utilities to obtain and review information regarding my consumption history from the Utility. This information may be used by Frontier Utilities and any of its affiliates, agents or representatives to determine whether it will commence and/or continue to provide energy supply service to me and shall not be disclosed to a third-party unless required by law. My electronic acceptance of this agreement shall constitute authorization for the release of the information to Frontier Utilities. This authorization will remain in effect during the Initial Term and any Renewal Term of this agreement. I may rescind this authorization at any time by providing written notice thereof to Frontier Utilities or by calling Frontier Utilities at 1-866-926-8194. The customer should review the contract and/or contact the current supplier to learn if any early termination fees are applicable.

I hereby authorize Frontier Utilities to obtain and review information regarding my consumption history from the Utility. I understand that I am allowed 3 business days after electronic acceptance to rescind my enrollment without penalty, and I may do so by calling Frontier Utilities customer care at 1-866-926-8192 between the hours of 8 a.m. and 5 p.m, Monday through Friday and 8 a.m. to 5 p.m., Saturday, Central Time (contact center hours subject to change without notice) or by writing to Frontier Utilities at the address provided in the Agreement.

I understand that I am allowed 7 calendar days following a confirmation notice from the electric utility to rescind my enrollment without penalty, and I may do so by contacting my electric utility. I also understand that I am allowed 7 business days following a confirmation notice from my incumbent natural gas company to rescind my enrollment without penalty, and I may do so by contacting my incumbent natural gas company.

I understand that I am allowed 10 calendar days following a confirmation notice from the electric utility to rescind my enrollment without penalty, and I may do so by contacting my electric utility.

I am the person with full name listed on the Sign Up For Service page or the legally authorized immediate family member of that person and I am not an agent enrolling on behalf of that person. I understand that if I misrepresent who I am including failing to identify whether I am the person’s agent, then the enrollment will be invalid.
